AGE-FRIENDLY COMMUNITY PROJECT - Communication Survey
The Age-Friendly Community Project Team is comprised of eleven (11) volunteers who reside in LCF and are helping develop a Master Plan to make our community more accessible and friendly for residents of all ages. The Project Team would like to hear from our community to learn what forms of communication work best for our community and you.  

This is a quick survey that will take approximately 3 minutes to complete.

This questionnaire is available in other languages.  Follow the instructions below:
  1. Open the form in Google Chrome
  2. Once your form opens, right-click on a blank part of the form (it won't work if you click on a form element). A pop-up menu should appear.
  3. Click the Translate to English option.
  4. When the language chooser pop-up appears, click the three dots icon and select Choose another language.
